After your visit, we will be happy to provide you with your clinic note and can send a copy to your referring provider per your request. This will include our assessment, any diagnostic findings, and plan of care.
We encourage you to use the My Health At Vanderbilt, our online patient portal, as a means of communicating with our staff.
Follow-up appointments will be made at the advisement of the autonomics provider. Please note that if a follow-up office visit is recommended, it will most likely be scheduled with the autonomics nurse practitioner.
It is imperative that you maintain your relationship with your primary care provider and referring provider as our clinic will only advise on matters related to the reason(s) you were referred to our clinic. We will not serve as your primary care provider or complete FMLA or disability paperwork. However, we are happy to provide your referring or primary care provider with the details of your visit so that they may complete any paperwork for you.
