Title of project: Electronic Photo Documentation to Manage Complex Wounds in Home Health
Why did you choose your project: Wound care is a challenge and increasing in complexity as home health nurses manage a variety wounds. Recent isolated occurrences influenced our decision to initiate the pilot.
- Nurses encountered patients with ulcers at start of care which were undocumented at hospital discharge.
- Nurses were managing more chronic, non-healing wounds.
- Anecdotal reports of inconsistent communication on wound status and wound measurements among nurses and with physicians.
