FAQ

  • All VUMC documents require approval by either the Patient Signature Forms Committee or the Medical Record Forms Committee. Approved forms that go in the medical record are accessed from Medex and contain Vanderbilt identifiers, doc type naming, an assigned VMR number and patient and document barcoding. Health Record Forms is responsible for assigning this information. Documents that have not gone through this submission and approval process will not be entered into the medical record.

  • Paper forms are retrieved from MedEx where they print with a patient and doc type specific barcode. Once the document is complted and signed, it will be scanned into OnBase. The barcode allows accurate indexing for faster availability in Epic.

  • VMR Forms and the forms approval committees will help guide your document in the appropriate direction. Some forms will be created natively in Epic, others will be designed electronically and some will remain in paper format.

  • After November 2nd, 2017, eDocs no longer contains medical record forms and only houses:

    • Reference Documents
    • Patient Education Documents
