The Family Medical Leave Act (FMLA) allows qualified employees to take up to 12 weeks of leave when they or certain family members have a serious health condition.

Qualifying family members include:

  • Spouse
  • Parent
  • Dependent child

Steps in the Vanderbilt FMLA Process:

  1. Notify your supervisor that you need to request medical leave.
  2. You or your supervisor submit the form from the FMLA HR website. (University employees use this link; VUMC employees use this link)
  3. The FMLA Coordinator determines if you are eligible for FMLA and sends you and your supervisor the Notice of Rights and Responsibilities and Medical Certification form.
  4. Complete your portion of the Medical Certification form and have your medical provider (or the provider for your family member) complete the medical information.
  5. The medical provider should submit the completed Medical Certification form to the fax number provided on the form.
  6. Once your Medical Certification form is received, it will be reviewed for a determination to be made if the condition meets the FMLA definition of a serious health condition. You will be contacted if more information is needed to make this decision.
  7. The FMLA Coordinator notifies you and your supervisor by email whether your medical leave determination was approved or denied.
  8. If approved, the FMLA coordinator will provide you and your supervisor an approval letter stating the provisions of your leave.
