You can make a referral as a patient, parent, or provider.
To make a referral:
- Provide the requested information in the link below.
- If you are a provider or parent, please have a Release of Information on file.
- Fax relevant medical records to 615-875-1401 (ATT: Kelly Vance).
A referral to Navigate does not guarantee acceptance into the program. We consider a myriad of factors (diagnosis, location, etc) when considering who will benefit from the program.
