County-Specific Summaries

The Rutherford and Williamson County summaries were drawn from the VUMC board-approved CHNA. The Davidson County summary is the Healthy Nashville Community Health Assessment, which is a Davidson County-specific report prepared by Metro Public Health Department in collaboration with VUMC and Ascension Saint Thomas Health.

Community Health Summits

VUMC'S Community Health Improvement Team hosted a Community Health Summit in each of the three counties - Davidson, Rutherford, and Williamson County. During the summits, the team presented data for the 2019 cycle of the CHNA and facilitated exercises to prioritize health needs. Explore the links below to access the data presentations and summit materials from each county.

2019 CHNA Instruments

Below are the instruments used to gather input from the community during the CHNA.

Environmental Scans

The 2019 CHNA methodology included an environmental scan of existing reports relevant to community health. The purpose of the scan was to review information released by community organizations to identify strengths, assets, and areas for improvement regarding health and healthcare in the community. The information gathered from these scans is summarized in the files below.

Additional Reports

Below are additional reports that are considered supplemental to VUMC's CHNA. These reports were completed as a part of NAACHO'S MAAP (Mobilized For Action through Planning and Partnership) process utilized by VUMC and its collaborators in Davidson County.
