We are very sorry that Shade Tree is not accepting new primary care patients at this time.
However, our prenatal services are always accepting new pregnant patients!
Shade Tree Clinic provides the following services free of charge:
- Primary care for adults
- Laboratory testing
- Medications
- Screening and diagnostic imaging
- Chronic disease management
- Nutrition counseling
- Vaccines
- Social services & patient advocacy
- Interpreter services
- Legal services (with thanks to the Middle Tennessee Medical-Legal Partnership)
Please note: If you are pregnant, we are still accepting new patients into our prenatal care program. Call 615-422-5262 for more information. The Vanderbilt Nurse Midwives at Melrose currently operates at the same location as Shade Tree. The map to the left shows other clinics in the community. Please contact us if we can help you find a clinic that fits your needs. We sincerely apologize for this inconvenience.
Prenatal Care
If you are pregnant, uninsured, and are not receiving any prenatal care, please contact us. We can help you get FREE prenatal care and social services.
Dispensary
We have an on-site dispensary to provide any necessary medications to our patients at the end of their appointment, free of charge.
Education Programs
Patient health educators provide education on a wide range of topics, including diabetes, high blood pressure, weight loss, medication management, diet, and exercise.
Social Work
Our social work team provides information to patients and referrals to our community partners for dental, vision, insurance, housing, food, mental health, legal, and early pregnancy resources.
Middle Tennessee Medical-Legal Partnership
Our collaboration with the Legal Aid Society of Middle Tennessee and the Cumberlands has created the Middle Tennessee Medical-Legal Partnership, allowing patients to access free legal services for needs such as immigration assistance, landlord-tenant issues, debt and tax disputes, accessing public benefits, employment issues, wills and guardianship, and school and discipline issues.
