Vanderbilt University Medical Center’s Goals of Care Initiative is driven by the intention of improving quality of life and health care received by patients with advanced illness and near the end of life. Physicians and other health care providers can play key roles in ensuring that every person receives care that is aligned with their stated goals and preferences.

This website will be a resource for clinical professionals to find up-to-date information about best practices, conversation guides, advance care planning tools, and upcoming educational opportunities. Aligned with VUMC’s commitment to high quality patient care across the lifespan, the end of life initiative is supported by VUMC’s Palliative Care Program, the Center for Bioethics and Society, and the Office of Health Sciences Education.
