Required Documentation for All Students
Before starting your clinical rotation at Vanderbilt, all students will complete the documentation requirements below. Forms must be submitted to the student coordinator listed for your clinical area below 8 weeks prior to your first day of fieldwork (10 weeks prior for Peds rotations). If you have any problems submitting your forms, please reach out to your student coordinator so there is no delay in starting your rotation.

- Background Check

Copy of Student and School Liability Insurance Forms

Copy of CPR Card (American Heart Association or the Red Cross)

Proof of 2 separate TB Skin Tests in the last year (most recent must be within 12 weeks of the start date)

Proof of current Flu shot (if rotation occurs between 10/1-3/31)

Proof of COVID-19 Vaccination and Booster
ALL STUDENTS ARE REQUIRED TO HAVE BOOSTER VACCINES

As part of the onboarding process, students must complete compliance and safety training for VUMC. You can access the modules by going to the VUMC Learning Exchange. Use your VUNet ID and password to log in.
VUMC's EMR is eStar (Epic). All students and new employees are required to take a training course on eStar/Epic. You will be notified of the time and location of your training. Prior to arrival, you may be asked to complete online prerequisites for eStar courses. The prerequisites will also be found in the VUMC Learning Exchange.
