EXISTING/OPEN CLINICAL TRIAL - Protocol Amendment/Revision 

If study protocol or imaging guideline amendments or revisions have occurred to a trial that is already open to accrual, please submit an existing/open trial amendment/revision review request via iLab. After full review, the trial's Clinical Trials Imaging Instructions (CTII) form will be revised if warranted.
