Please Report COVID-19 Exposures Outside of Work to Occupational Health

Keeping yourself safe always includes wearing your mask, maintaining social distancing and washing your hands. It also includes monitoring yourself for signs of potential infection.

If you have an exposure outside of work, it is still important to inform Occupational Health using this link https://www.vumc.org/health-wellness/news-resource-articles/2019-novel-…, so they can provide guidance as to any specific quarantine or additional masking needs and monitor you for the development of any symptoms. You should also use this form if someone close to you test positive for COVID-19.

If you are tested outside of the Vanderbilt system and the test is positive, please notify Occupational Health by calling 615-936-0955 and speaking to one of the clinicians. They can provide return-to-work guidance, answer any questions, and do appropriate tracing for any potential contacts at VUMC.
