Metal Screening Form
The metal screening form is a tool that we use to determine whether or not a patient is safe to scan. The link can be found below. This form goes over past surgical history. It is very important that you complete the form with ALL surgeries starting from childhood through the present. Even surgeries that seem minor must be listed on this form (like wisdom teeth or tonsil removal). When you finish the first page of the form, hit the save and return later button found at the bottom. This will generate a code of letters and numbers. Please send this code to the clinical trials specialist for the study you are participating in.
