Waiving Gallagher Student Insurance

 

Spring 2018 Waiver Information

1.  Are you a student that will be new to Vanderbilt in Spring 2018?     Newly enrolled students for the Spring of 2018 who wish to waive the Vanderbilt Student Insurance will have to complete a waiver form and enter comparable coverage to be considered for the waiver.  Students who do not waive the Gallagher Student Health insurance plan will be enrolled in the plan and billed for the premium through Student Accounts.   Deadlines for Waiver-- December 1, 2017 through January 4, 2018Petition to waive after the deadline will also be considered from January 5, 2018 through February 15, 2018.  Follow this link to complete the waiver: www.gallagherstudent.com/vanderbilt.  

Your insurance coverage submitted for the waiver request must meet the following guidelines:

  1. Be provided by an insurance carrier that is based in the United States. International policies, travel insurance and short term medical policies are not acceptable.
  2. Comply with all provisions of the Affordable Care Act. Policies that are comparable to the Affordable Care Act are not accepted.
  3. Provide coverage from August 12, 2017 to August 11, 2018.
  4. Provide an unlimited plan maximum benefit. Plans with maximums on a per illness, per injury, per coverage period, or per lifetime basis will not be accepted.
  5. Have In-Network Providers located in Nashville, TN.
  6. Have an In-Network deductible no greater than $150 per policy year for Graduate and International.
  7. International student plans must exceed the Vanderbilt plan and the coverage for coinsurance must be at least 85% In-Network.
  8. Have an In-Network deductible no greater than $250 per policy year for Undergraduates
  9. Provide immediate coverage for Pre-existing Conditions; no waiting period for services.
  10. Provide coverage for Maternity Coverage and Newborn Care.
  11. Provide coverage for Mental Health/Substance Abuse with no limits on outpatient or inpatient services.
  12. Provide coverage for prescription drugs. .
  13. Provide coverage for Preventive Care Services. This means coverage for routine physicals; physician visits exams; gynecological exams; and coverage for preventive laboratory tests, immunizations, and vaccinations.
  14. Provide coverage for Emergency Room Services.
  15. Contain Worldwide Travel Assistance, including unlimited medical evacuation and repatriation of remains.

 

2.  Are you a current Vanderbilt graduate or professional student and will be graduating in December 2017?      If you would like your current Gallagher Student Health Insurance plan to be canceled as of December 31, 2017, please see the form here and follow the instructions.  The deadline for submission of the form is December 31, 2017 and no late forms will be accepted.

 

3.  Are you a current Vanderbilt student that is enrolled in the Fall 2017 semester and will continue in the Spring 2018 semester?     If you are enrolled in the Annual Gallagher Student Health Insurance plan that started August 12, 2017, and you wish to dis-enroll from the Spring and Summer 2018 portion of the plan, then you should proceed to the Gallagher Student Health Insurance website at www.gallagherstudent.com/vanderbilt and complete a spring semester disenrollment request form.  You will need to provide insurance information that demonstrates adequate coverage while you are a student at Vanderbilt University.  The deadline for submission of this form is January 15, 2018. No late waivers will be accepted.
