VUMC employees who enter a patient’s room with someone who is not wearing a mask are protected if they are following VUMC guidance: wearing a surgical mask, wearing eye protection, and practicing hand hygiene. Surgical masks protect against droplets being shed by the person wearing the mask AND against breathing in droplets shed by someone else. As a reminder, for other serious infections spread by droplets, such as contagious meningitis, health care personnel have depended on surgical masks for personal protection for years, even when the contagious patient is not wearing a mask.</p> <p>VUMC policy requires all people to wear masks (unless exempted by medical condition or age &lt;2 years). VUMC employees should ask visitors in the room to please put their mask on while in the room caring for the patient. Here is some suggested language for how to address it with a patient or visitor:</p> <ul> <li>Don’t forget to put on your mask. It’s one of the best things we can do to keep ourselves and others safe.</li> <li>I see that you’re not wearing your mask. The external valves/vents are designed to release unfiltered air and do not effectively remove the virus from exhaled breath in the event the wearer has COVID-19 infection. This vent does not impair the device’s filtration of inhaled air when worn as personal protective equipment to protect the wearer. For VUMC:</p> <ul> <li>A face mask with an external valve/vent should not be used. For some persons with severe chronic lung disease, wearing a mask may make breathing more difficult, but not because of CO2 retention.</p> <p>Recall, there are health care workers who routinely wear masks for prolonged periods as part of usual care (such as when performing surgery) and adverse effects from this practice have not been reported.</p> <p>Cloth masks reduce the number of microorganisms that someone releases into the air. Certainly, people at greatest risk for spreading COVID-19 are symptomatic people. We need to remember that before symptoms begin, during a period when infected people are “presymptomatic,” they can shed significant amounts of SARS-CoV-2 virus and infect others.</p> <p>A minority of infected persons never develop symptoms and are classified as “asymptomatic.” These persons still shed virus, but not as much as symptomatic (or presymptomatic) individuals do. It is important to keep in mind that asymptomatic, infected people are more likely than symptomatic persons to be out in public conducting business as usual, in situations where they might transmit the virus to others.</p> <p>Infected persons without symptoms (both asymptomatic and presymptomatic) look and feel like their usual selves.
